Required Education – Lindsay’s Law

If you did not turn in a signature form with your registration, please print and sign the form below (parent and player).  Once you have signed the form, please scan and email it to [email protected].  You can also opt to bring the form to your first practice and provide to your coach.  All players must have signed form on file to participate.  Send any questions to [email protected].

 

Signature Form: Parent-Athlete-Signature-Form

In order to comply with current state law, all coaches, players 19 and under, and players’ parents in the BBSYL must view the applicable Lindsay’s Law video, read the Lindsay’s Law Information sheet, and sign the Lindsay’s Law Signature Form prior to participating in league activities.

In accordance with this law, the Ohio Department of Health, the Ohio Department of Education, the Ohio High School Athletic Association, the Ohio Chapter of the American College of Cardiology and other stakeholders jointly developed guidelines and other relevant materials to inform and educate students and youth athletes participating in or desiring to participate in an athletic activity, their parents, and their coaches about the nature and warning signs of sudden cardiac arrest.
